City Clerk: Birth and Death Registrations

Registration of all Births Occurring in the City of St. Catharines

The Statement of Live Birth (FORM 2)
Parents of newborns will receive a form called “Statement of Live Birth” (Form 2) from the hospital or Midwife where the baby was born. This form must be completed and returned in the envelope provided in order for your child’s birth to be registered.. Instructions are provided on the application form for proper completion.

The Statement of Live Birth should be forwarded within 30 days of the date of birth to the Office of the Registrar General at the address indicated below.

How to Amend a Birth Registration
If a birth has been registered in Ontario and the parent(s) wish to amend any information recorded then an application must be made to the Office of the Registrar General. Please contact them by calling 1-800-461-2156 and the forms will be sent to you.

How to Obtain Old Birth Records
The Office of the Registrar General holds records for births that happened in Ontario during the past 95 years. For information on older records you should contact:

Birth and/or Death Certificates

Applications to obtain birth or death certificates for any province in Canada may be obtained from the City Clerk’s Office third floor, or the Information Desk on second floor. Applicable fees charged by the Province are set out on the application form. The completed application form, together with the appropriate fee, must be mailed to the Office of the Registrar General, 189 Red River Road, P. O. Box 4600, Thunder Bay, Ontario, P7B 6L8.
